Targeted Case Management Referral Form
WHO QUALIFIES?
Mental health targeted case management is available to Medicaid recipients who:
– Do not receive case management through a home or community-based waiver;
– Are not in hospitals, nursing homes, intermediate care facilities for the developmentally disabled, or mental health institutions;
– Adults: who are 18 or older and require advocacy or service coordination to maintain or improve functioning;
– Adults: 18 or older, living in the community, and at risk of long-term hospitalization due to mental illness without ongoing interventions.
