• Individualized care plans created using person-centered, strengths-based approaches
• Connections to vital resources, including health care, housing, food, and clothing
• Help with applications for Public Assistance, Unemployment, Social Security, and other benefits
• Education and information on health topics such as vaccinations, COVID-19, and preventive care
• Support in overcoming barriers like transportation, legal aid, and obtaining identification
• Collaboration with community partners to ensure smooth and coordinated service delivery
• Active participation in community meetings and coalitions to strengthen the provider network
